Early Intervention
Early Intervention
Service Coordination is available through CCR for children ages
birth through three years, who have a developmental delay or a
medical diagnosis which may put them at risk for having a diagnosis
of Mental Retardation later on.
A Service
Coordinator will assist families in developing an individualized
family support plan which outlines therapy providers and services.
Service coordinators help families locate community resources and
monitor services to ensure that both child and family needs are
being met.
Any child between
birth and three years can be referred for Early Intervention. The family
will have an intake scheduled with an EarIy Intervention Service
Coordinator and be referred to the county’s independent team for a
comprehensive developmental evaluation.
If a child is found
to have a delay of 25% or greater in any area of development (motor/
cognitive/ speech and language/ self-help/ social-emotional) the
family will be offered the appropriate Early Intervention services
to help remediate the delay.
