Mental Retardation


 

The MR Program provides for Supports Coordination case management that link individuals with an MR diagnosis and their families to community activities, services, and supports they would like to receive. 

 

Services include:  Case Management; Individual/Family assistance in navigating the MR System; Medicaid Home and Community Based Waiver Program application; and/or completing referrals/applications for client service choice (employment, residential placement, day/vocational activities, respite, and/or equipment needs).

 

Eligibility

  • Documented significant sub average intellectual functioning (Full Scale IQ - 70 or below) with onset prior to the individual’s 22nd birthday;

  • Deficits (at least 2) in the ability to perform daily living skills (self-care, home-living social/interpersonal skills, use of community resources, self direction, functional academic skills, work, leisure, health, and safety).

Referral Process

  • Contact the Center for Community Resources, Inc., 220 S. Main Street, Suite 101, Butler, PA 16001, 724 431-0095 and ask for the Information and Referral Department.

  • Documentation in support of eligibility for services (medical, psychological and/or school records that indicate an MR diagnosis—Full Scale IQ of 70 or below with onset prior to the individual’s 22nd birthday).
