Service Coordination
Service Coordination
Many people who use human services are involved in more than one system. These systems have eligibility requirements, rules and regulations that are often confusing. As Butler County's Base Service Unit (BSU), Center for Community Resources ensures that theservices available to individuals with mental health or substance abuse needs are coordinated in the best interest of the person. We have a team of Service Coordinators who collaborate, coordinate and make referrals to providers from various systems (Mental Health, Drug & Alcohol, Criminal Justice, Hospitals, Employment agencies, Medical Assistance and other government agencies).
Service Coordinators:
-
Provide a thorough clinical assessment of the individual which will be used to help identify strengths and needs
-
Identify providers of desired services in requested locations
-
Help organize services so they are easy for individuals and families to use
We provide information and referral, assessments and linkages to resources within Butler County. We are here to connect, coordinate and offer you ongoing guidance and support as you navigate through the Butler County human service system.
