Case Management & Service Coordination

Our Service Coordinators help people with a mental health disorder and/or substance abuse needs that have potential or current involvement in the criminal justice system.  Service Coordinators provide assessments, referrals, linkages to various resources, and coordination of services. Service Coordinators assist people in accessing the following resources:
 

Mental Health/Drug &Alcohol treatment Psychiatric services Housing services Employment services Potential employers Vocational services Accessing public benefits (Medical Assistance, Food Stamps, etc) Transportation

Our goal is to connect people to services that support and promote living in the community without future involvement in the criminal justice system.
