Homeless Case Management

Everyone deserves safe, affordable housing regardless of their special needs. Our Homeless Case Management Program provides permanent supportive housing assistance for chronically homeless, disabled consumers (over the age of 18) with a mental health disorder and/or struggles with substance abuse. Housing is provided at various apartment locations.
