Supports Coordination

What is Supports Coordination?

Supports Coordination is a service that helps individuals with intellectual disabilities and/or autism (ID/A) locate, organize, and manage the supports they need to live healthy, safe, and meaningful lives in the community.

 

This service is overseen by the Administrative Entity (AE) at the county level, the PA Department of Human Services and the Office of Developmental Programs.

How can a Supports Coordinator help me?

  • Using a person-centered approach and team process, we help individuals with disabilities develop a service plan focusing on their strengths, goals, and what’s important to them.

  • Participants and their families are informed of services and supports available to them; we then assist in the selection of qualified providers.

    Once a provider is chosen, we connect the participant with the provider via a referral. We offer continued follow-up support to all parties.

  • We visit with the participant and their formal and informal supports to ensure that the chosen providers and service plan meets the participant’s needs.

  • Through the promotion of community integration, we can meet the participant’s needs in the least restrictive manner.

  • We respond to and assess emergency situations and incidents to ensure appropriate actions are taken to protect the participant’s health and welfare.

To obtain a Supports Coordinator?

Call the Allegheny Office of Developmental Supports at 412-253-1250. They will determine your eligibility, perform an intake, and give you choice of Supports Coordination Organizations.

To switch my Supports Coordinator to CCR?

Contact your current Supports Coordination Organization and ask about an SCO transfer to CCR.
