Butler County Social Determinants of Health

CCR's SDOH Care Managers serve as a liaison between behavioral and physical health, social services, and the community to improve access to and coordination of resources.

We help with:

  • Substance Use

  • Transportation

What to Expect

The Care Manager will:

  • Greet you in-person or by phone.

  • Perform an intake to determine your areas of need.

  • Provide you with appropriate referrals and linkages to meet your identified needs.

  • Provide scheduled follow ups to ensure referrals and linkages were successful.

How Does the Program Work?

  • Provides support to individuals navigating the health and social services systems.

  • Conducts health and social needs screenings while educating individuals on prevention strategies.

  • Initiates referrals and coordinates health and community services.

  • Advocates for individual and community needs.

  • Provides direct services such as advocacy and service coordination.

  • Builds individuals' ability to achieve wellness and increases the community's ability to address social determinants of health.
