Blended Case Management Services

What is Blended Case Management (BCM)?

Blended Case Managers build on individuals’ strengths and natural support systems, connecting them with essential services and resources to support physical and mental well-being, foster resilience, and enhance overall quality of life.

To qualify for BCM services, you must:

  • Be five years old or older; AND

  • Live in Clinton or Lycoming County; AND

  • Have a documented mental health diagnosis.

CCR’s BCMs can assist with:

  • Connecting individuals to community resources such as housing, food, and transportation.

  • Coordinating mental and physical health services.

  • Supporting access to education or employment opportunities.

  • Helping individuals navigate complex systems such as healthcare, social services, and public benefits.

  • Teaching skills for daily living, including budgeting, medication management, and communication to promote self-sufficiency.

  • Supporting individuals and their families in their mental health recovery journeys.
